What Digital Smile Design Really Is <a id="what-dsd-is"></a>

Digital Smile Design, diseño digital de sonrisa in Spanish, is a clinical protocol developed by Dr. Christian Coachman and refined by contemporaries including Dr. Edward McLaren, Dr. Frank Spear, and Dr. John Kois. At its core it is a disciplined method for planning aesthetic dentistry by starting with the face, with facial photography, facial scanning, and facial dynamics, rather than with the teeth.

This inversion matters. For most of the twentieth century, aesthetic dentistry planned teeth in isolation. A clinician would examine the teeth, prescribe veneers, and the laboratory would fabricate them to textbook dimensions. The result was often aesthetically disappointing in the face even when it was beautiful in the intraoral mirror, teeth that were too long, too light, too uniform, too much like a magazine and not enough like the specific face they had to harmonise with.

Digital Smile Design corrects this by making the face the starting reference. We capture the face at rest, smiling, laughing, talking. We capture lip dynamics. We capture the interpupillary line. We capture the relationship between the upper central incisors and the lower lip line at repose. Only then do we design the teeth, to harmonise with the face, not to override it.

At Stunning Dentistry, every smile design case that involves more than two anterior teeth includes a full face-scan-led protocol. For smaller cases, one or two teeth, we use the simpler Coachman 2D photographic protocol. For full-mouth aesthetic rehabilitation, we extend to full facial 3D scanning with Bellus3D or FaceHunter and dynamic recording with Modjaw.

The Face-Scan-Led Protocol (Coachman, McLaren, Spear, Kois) <a id="face-scan-led"></a>

Our protocol integrates elements from four published aesthetic frameworks. We credit each source.

Coachman Digital Smile Design (2007, extended): 2D photographic analysis, ruler overlays on facial photographs, integration with dental wax-up.

McLaren's Dentofacial Analysis (2015): systematic framework for facial midline, occlusal cant, smile arc, buccal corridor, gingival display, the vocabulary we use in our clinical record.

Spear's Facially Generated Treatment Planning (2018, extended): integration of aesthetic planning with functional and biological considerations; the "centric-relation-versus-maximum-intercuspation" decision captured in the plan.

Kois Framework (ongoing): four-diagnosis system (aesthetic, functional, biological, periodontal) applied to every smile design case.

The technical stack supporting this protocol:

The Material Menu, Composite to Zirconia <a id="material-menu"></a>

Digital Smile Design is not a material; it is a planning protocol. The materials we choose, based on diagnosis, range from the most conservative to the most extensive.

Direct composite bonding. The most conservative option. Resin composite placed directly onto enamel with minimal or no tooth preparation. Good for minor chips, small gaps, slight colour corrections. Not long-term for wear, stain, or marginal discolouration. 5-year warranty.

Feldspathic porcelain veneers. The traditional high-aesthetic veneer. Extremely thin (0.3-0.5 mm), layered by skilled ceramists, highest aesthetic potential but highest technique sensitivity. Conservative tooth preparation. 10-year warranty.

Lithium disilicate (e.max) veneers or crowns. The modern workhorse. Stronger than feldspathic, excellent aesthetics, predictable. Our most common choice for Spanish aesthetic cases. 10-year warranty.

Ultra-thin zirconia veneers (Straumann Zirlux, Prettau). Newer generation zirconia thin enough for veneer thickness with zirconia's strength. Good for bruxers. 10-year warranty.

Full-coverage zirconia crowns. When prior restoration or structural tooth loss prohibits veneers. 10-year warranty.

Orthodontic repositioning before restoration. Where alignment is the core issue, not colour or shape, we recommend orthodontics first and may skip veneers entirely. Aligned natural teeth beat veneered crooked teeth every time.

Combination treatment. Often the right answer, orthodontics plus limited veneers plus composite bonding, rather than 20 veneers.

Why We Decline to Up-Sell Veneer Counts <a id="no-upsell"></a>

The single largest aesthetic-dentistry complaint received by European regulators, the COEC in Catalonia, the Consejo General in Madrid, and their counterparts elsewhere, is over-treatment. Patients presenting with concerns about one or two anterior teeth are quoted ten-veneer or twenty-veneer reconstructions. The clinic's financial incentive runs in one direction; the patient's dental health in another.

At Stunning Dentistry, we decline to up-sell veneer counts. Our policy:

• If your concern is one tooth, we treat one tooth
• If your concern is two to four anterior teeth, we treat two to four
• If your concern is a full anterior aesthetic result that legitimately requires six to eight teeth (typically canine to canine or first premolar to first premolar), we explain why and show you the wax-up before you commit
• We refuse to recommend 14 or 20 veneers unless the functional and biological diagnosis supports it, which in our Spanish caseload is perhaps 6% of aesthetic cases

We will tell a Spanish patient who has received a 10-veneer quote from a Madrid or Barcelona clinic that they probably need four veneers, or sometimes two, or sometimes just orthodontics and a composite bonding. This sometimes costs us the case. It has kept our warranty numbers low and our reputation intact.

What This Costs in EUR <a id="cost-in-eur"></a>

Inclusive of: full Digital Smile Design protocol, facial 3D scan, Modjaw recording, exocad design, 3D-printed mock-up, milled PMMA provisional, definitive ceramic restorations, 3 on-site post-op reviews, 3-month video follow-up, annual video review for the warranty period.

Not included: pre-existing periodontal treatment, endodontics on teeth not scheduled for aesthetic treatment, orthodontic phase if recommended separately.

Insurance for Spanish patients: Aesthetic veneers are typically not covered by Spanish private insurance. Full-coverage crowns that replace failing restorations may be partially covered. Sanitas, Adeslas, DKV, Mapfre Salud, Asisa, we supply itemised EUR and INR receipts plus Spanish clinical summary.

For Spanish Patients: Trip Plan and Home Dentist <a id="spanish-logistics"></a>

Trip length. 10-14 days for a 6-10 tooth aesthetic case. 5-7 days for a minor case (2-4 teeth).

Flights. Iberia, Lufthansa, Turkish Airlines, Qatar Airways, Etihad from Madrid (MAD), Barcelona (BCN), Valencia (VLC), Seville (SVQ), Bilbao (BIO), Málaga (AGP), Zaragoza (ZAZ), Palma (PMI).

Hotel. Eight partner properties; for smile design cases with substantial photographic documentation we typically recommend properties with good natural light in the rooms (Park Hyatt, ITC Kohenur, Taj Krishna).

Language. Consultations in English. Anjali Reddy provides Spanish support during Madrid business hours. Aesthetic consultation transcripts available in Spanish.

Home dentist handover. Full record, facial scans, intraoral scans, wax-up files, shade photographs, restoration specifications, transferred to your Madrid/Barcelona/Valencia/Seville/Bilbao/Málaga/Zaragoza/Palma dentist.

Aftercare, Warranty, and the 10-Year View <a id="aftercare"></a>

Weeks 1-4: soft-to-normal diet return. No biting into hard fruits like apples; slice them. Gentle brushing of ceramic-restored teeth. Professional hygiene at 3 months.

Months 3-12: normal function. Annual hygiene with your Spanish dentist. One-year photographic review with us via video.

Years 1-5: standard dental hygiene. Small repairs to composite bonding may occur; covered under 5-year warranty.

Years 5-10: some wear expected on feldspathic; lithium disilicate and zirconia largely stable. Warranty covers fracture under normal function.

Warranty categories: A 8.7%, B 2.4%, C 1.5%, D 0.3%. Same distribution across our 924-case dataset. Escalation same as FMR pathway; with aesthetic-specialist independent consultant in Barcelona for dispute resolution.

What We Will Not Promise <a id="wont-promise"></a>

We will not promise a result that matches a magazine photo. Your face, your smile, your soft-tissue contour, and your occlusion are specific to you. We design to harmonise with your face, not a generic template.

We will not promise that every one of your friends will immediately notice. Good aesthetic dentistry is often subtle, the teeth look like they belong to you, the way they always should have.

We will not promise that aesthetic dentistry solves underlying functional or biological problems. If you grind your teeth, you will grind your veneers. If your gums are unhealthy, veneers will not fix that. We address the underlying issues first, or decline the case.
