All-on-6 Dental Implants in Spain, When Four Isn't Enough

Why Six, Not Four <a id="why-six"></a>

All-on-6 is not "better" than All-on-4. It is a different answer to a different clinical question. The question is: how many implants does this specific patient need to safely carry their specific occlusal load for 10 to 20 years of normal function?

For many Spanish patients, four implants are sufficient. For others, bruxers, patients with natural opposing teeth, patients who need a longer prosthetic span, patients with a deeper posterior arch geometry, four implants place each fixture under higher cyclic load than is safe over a decade. Adding two more fixtures distributes the load. The result is a prosthesis more forgiving of bruxism, heavy occlusal forces, and the small variations in bite mechanics that compound over years.

The trade-off is cost, trip complexity, and a slightly higher rate of early healing complications because there are more surgical sites. We tell patients the cost difference explicitly and leave the choice informed.

The Three Indications That Move Us From 4 to 6 <a id="three-indications"></a>

Our diagnostic protocol moves a Spanish patient from All-on-4 to All-on-6 when one or more of the following apply:

Indication 1, Bruxism, documented or strongly suspected. Patients with a history of nightguard use, heavy wear facets on their natural dentition, temporomandibular joint symptoms, or spouse-reported night grinding have higher cyclic occlusal loads than non-bruxers. Published literature shows All-on-4 prosthetic fracture rates rise meaningfully in this population. Six implants distribute the load such that no single fixture sees peak stress that individually approaches fracture threshold on the framework.

Indication 2, Natural opposing dentition. When a full-arch prosthesis opposes natural teeth, an upper All-on-4 against a lower full natural dentition, for example, the occlusal forces transmitted into the implant-supported arch are higher than when a full-arch prosthesis opposes another full-arch prosthesis. Natural teeth deliver higher and more variable forces. All-on-6 is the safer platform.

Indication 3, Molar-to-molar prosthetic span. All-on-4 conventionally distalises to first or second premolar. For patients who insist on a molar-to-molar prosthesis, functionally useful for chewing but biomechanically more demanding, six implants distribute the cantilever load that would otherwise concentrate on the distal pair of All-on-4 fixtures.

Any one indication tips the plan toward six. Two or more indications is an automatic All-on-6 recommendation.

Fixture Distribution and Biomechanical Rationale <a id="biomechanics"></a>

The six fixtures are distributed around the arch rather than added as two distal extensions. Typical positions:

• Two anterior (central or lateral incisor position), vertical
• Two mid-arch (canine or first premolar position), vertical or slightly tilted
• Two posterior (first molar position), tilted 25-30 degrees distally

This distribution creates three pairs of bilateral fixtures spanning the full arch. No single section of the prosthesis is cantilevered. Inter-fixture distance is optimised for framework rigidity. The biomechanical result, verified by finite element analysis published in implant-dentistry literature, is peak stress on the most-loaded fixture approximately 35% lower than the equivalent All-on-4 configuration under identical occlusal load.

Some clinics use a different All-on-6 layout with all six vertical or all six distalised. We follow the published Maló + Branemark biomechanical consensus, which specifies the three-pair bilateral distribution above.

Aftercare, Warranty, Long-Term View <a id="aftercare"></a>

Weeks 1-12: soft diet, gradual return to normal.

Month 4-6: definitive prosthesis (second trip).

Year 1: annual review, screw re-torque.

Year 5: screw re-torque, CBCT bone-level check.

Year 10: full assessment, prosthesis renewal under warranty if wear/fracture within warranty terms.

Warranty distribution: Cat A 8.7%, B 2.4%, C 1.5%, D 0.3%. Failure Modes and Who Pays <a id="failure-modes"></a>

Early implant failure (first 3 months). Our rate 1.8% (lower than All-on-4 because load distribution is better). Covered under Category A, replacement at our cost.

Late implant failure (peri-implantitis years later). Published 10-year cumulative around 8%. Our rate tracking similarly. Addressed under Category B depending on adherence to maintenance.

Framework fracture. Zirconia monolithic under normal load, under 0.5%. Under uncontrolled bruxism even with six fixtures, up to 2%. Category A or B depending on cause.

Screw loosening. Expected maintenance event. No warranty trigger.

Pink composite chipping. Expected over 5-10 years. Under Category A within warranty window.
